Crooked Willow Designer Series Application
Thank you for your interest in the Crooked Willow
Designer Series Program. In order for us to complete your
qualification, the following credentials must be submitted
via mail or fax to:

The Crooked Willow Designer Series
Program is for qualified applicants only.
To apply, you must be an interior designer
or merchandising specialist. The following
credentials are required for admittance:
Current retail business license or federal
resale tax certificate
Copy of your business check
Copy of your corporate credit card or proof of
merchant credit card ID#
Photo ID (one per buyer) or passport
Three references (name, address, phone number,
email)including clients and vendors
Please allow 7 -10 days for credential
verification. You will receive an email
confirmation of approval of credentials.
Your membership cards will be sent
out at that time.
